EACH THERAPIST WILL HAVE THEIR OWN FORM WITH PRICES AND HOURS SPECIFIC TO THEIR MODALITY POSTED ON DOOR FOR WALK BY, SELF SCHEDULING

$50 PER DAY (Kelleys Island) / OF THE WEEK (DUE AT 1ST OF MONTH IN ADVANCE WITH 5 MONTH SEASONAL CONTRACT)
EXAMPLE: If you choose Monday = $50 x 4 (or number of mondays in the month) is $200 for the month. If you choose Tues and Sat. = $50 x 8 (total number of Tues and Sat in the month) is $400 which is due May 1, June 1, July 1, Aug 1, Sept 1

YOUR PERSONAL THERAPY SPACE IS WALK-IN AND INCLUDES ONE ADJUSTABLE MASSAGE TABLE & BATHROOM. You will schedule your own clients or walk-by’s can schedule themselves on posted sign up sheet.

MUST PROVIDE YOUR OWN SHEETS/LAUNDERING
and Proof of Malpractice Insurance (if applicable), Proof of Licensure
